PARTICIPANT CONSENT, RELEASE & WAIVER OF LIABILITY

Event Dates: July 28 – August 3, 2025
Location: Woodie’s Flat, 1535 N. Wells St. Chicago, IL 60610

READ CAREFULLY – THIS IS A LEGALLY BINDING DOCUMENT

I, the undersigned, hereby acknowledge, understand, and agree as follows:

1. Voluntary Participation

I am voluntarily entering the Wing Week Challenge hosted by Woodie’s Flat (the “Event”). I understand this is a timed food-eating competition and participation is not required for purchase or general entry to Woodie’s Flat.

2. Event Description

Participants will attempt to consume as many Woodie’s wings as possible in 30 minutes. All wings must be sauced, with sauce chosen by the participant. Woodie’s Flat will have sole discretion to determine:

• The number of wings officially and successfully consumed
• The timekeeping and final tally
• Any disqualifying behavior or rule violations

3. Prizes

Prizes will be awarded to the top three (3) individuals who consume the most wings in 30 minutes:

• 1st Place:
  • $500 cash,
  • Free wings for one (1) year, defined as a maximum of one (1) single order of wings per calendar day, from July 29, 2025 to July 28, 2026, dine-in only, non-transferable, no cash value.
• 2nd Place:
  • Two (2) general admission tickets to Riot Fest 2025 (3-day pass)
  • One (1) $100 Woodie’s Flat gift card
• 3rd Place:
  • Two (2) tickets to a Chicago Cubs game in 2025 (game selected by Woodie’s Flat)
  • One (1) $50 Woodie’s Flat gift card

Woodie’s Flat reserves the right to substitute prizes of equal or greater value, at its sole discretion. Prizes are non-transferable and not redeemable for cash.

4. Health Risks & Assumption of Risk

I understand that competitive eating may involve serious health risks including, but not limited to:

• Choking
• Gastrointestinal distress or rupture
• Allergic reactions
• Burns (including from spicy sauces)
• Aspiration, vomiting, or other physical injury
• Death

I represent that I am in good health, have no medical condition that would put me at risk by participating, and have consulted a physician if uncertain. I voluntarily assume all risks—known and unknown—associated with participating in this Event.

5. Release of Liability

In consideration for being allowed to participate, I hereby release and discharge Woodie’s Flat, its owners, employees, sponsors, affiliates, agents, insurers, successors and assigns from any and all claims, demands, actions, liabilities, or damages arising out of or related to my participation in the Event, including but not limited to claims of negligence, illness, bodily injury, or death.

6. Indemnification

I agree to indemnify and hold harmless Woodie’s Flat and its affiliates against any third-party claims related to my conduct or participation in the Event.

7. Medical Treatment Consent

In the event of a medical emergency, I authorize Woodie’s Flat and its staff to seek emergency treatment on my behalf and accept full responsibility for any resulting costs or expenses.

8. Media Release

I grant Woodie’s Flat full permission to use my name, likeness, image, voice, and any footage or photos taken during the Event for promotional and marketing purposes in any media, without compensation or further approval.

9. Acknowledgment of Sound Mind

By signing below, I affirm that:

• I am at least 18 years old,
• I am of sound mind,
• I am not under the influence of alcohol or any substance that impairs my ability to make sound decisions.

10. Governing Law & Venue

This Agreement shall be governed by the laws of the State of Illinois. Any disputes arising hereunder shall be brought exclusively in the courts located in Cook County, Illinois.
